  3. Vaccination Records: It’s essential to have all vaccination records, including childhood immunizations, with you. Please ensure they are translated into English.
  4. I-693 Form: You have two options for the I-693 form:
    • You can bring a completed form, following the instructions provided by USCIS.
    • Alternatively, we can provide you with the form at our office.
  5. Included Services: Any necessary labs and X-rays required for the immigration medical exam are included in the package.
